| 1 | Company | 5 | Total hours entered in the timesheet for the employee | 9 | Hours allocated to the employee | 13 | Total of all values selected in the Modified calculated hrs column  | 
| 2 | Period number | 6 | Date of holiday identified according to the employee’s calendar | 10 | Amount allocated to the employee | 14 | Total of all calculated amounts associated to the values selected in the Modified calculated hrs column  | 
| 3 | Name of employee | 7 | Number of adjusted hours for the holiday according to the employee’s calendar and the type of hours determined in the company’s parameters | 11 | Value entered for the employment insurance earnings | 15 | Total of all entered values |
